Required Forms

All required forms you will need throughout your graduate studies are in Graduate Rebel Gateway → Forms.

If you would like to preview everything you may encounter, browse the full list of Graduate College forms. That directory is the Graduate College's catalog of student forms, and it indicates how each is submitted. Forms are program- and degree-specific and are routed for the required approvals, so use the directory to locate the correct form and its submission channel before starting any administrative step.

Forms are completed sequentially and must all be submitted and approved by the end of your last semester to ensure timely degree conferral.

Hold off on submitting your forms until your final or next-to-last semester, unless you are on the Professional Paper subtrack.

By default, everyone is in the capstone experience subtrack (see Culminating Experience).

If on a Capstone Experience or Internship subtrack

Plan of Study Parts I and IIBefore your oral defense, and after you discuss it with the Graduate Coordinator
Culminating Experience ResultsImmediately after you defend your professional paper (talk to the advisory committee chair first)

Plan of Study Parts I and II Form

It is encouraged that you plan out your courses in advance to build a tentative plan of study and discuss it with the Graduate Coordinator.

Practical purpose of this form

The Plan of Study Form is more a record of your study than a plan. Its real purpose is to communicate to the Graduate College which courses they should use for computing your program GPA and for checking your degree requirements.

Hold off until the beginning of your final semester to submit it. Run it by the Graduate Coordinator before submitting.

  • Part I of the Plan of Study Form can be found in the Graduate Rebel Gateway.
  • Part II is in the Degree Directory. Scroll to the "Documents/Downloads" section at the bottom of the page and look under either "Post-Bachelor's Track" or "Accelerated Track" (whichever applies to you) for the Plan of Study Part II forms, which are fillable Word files. You may use the form from any academic year in which you were an active student.
